To schedule an appointment, simply contact the location nearest you. Out of state and international patients coming by commercial air may find the Indianapolis or Fort Wayne centers more convenient as major airports are located nearby. Click to access out of state travel and lodging information.

You may e-mail questions to our doctors or contact us by phone. Our main office is located in Hartford City.

If coming from out of state we do recieve a special rate for patients from Jameson Inn. They also offer a shuttle that can take you directly to our office. To learn more download this PDF.
